Out of Service Notification
If you need to report a piece of Division equipment as out of service, please complete the form below. You may also be required to physically tag the equipment with an out of service tag as well to ensure that it does not get mistakenly used prior to being fixed or repaired.

This form is to be used to submit any major event within the Division. This could be a student or staff injury, accident or any major facility or equipment being out of service.
